Age 95 a resident of Frankfort, IL, former owner of Kozak's Tavern, member of International Polka Association (IPA) and American Racing Pigeon Union (ARPU), passed away on Tuesday, October 4, 2016. Beloved husband of the late Irene A. Kozak (nee Sebonia); loving father of James R. Kozak, Linda (Peter) Donzelli, and Michelle (Carmen) Ruffino; cherished grandfather of Ed Ambrose, Jennifer Kozak (Waqas Akkawi), Allison (Bryan) Barham, Joanna, late James, Sophia and Thomas Ruffino; and great grandfather of Ethan, Madison, and Aidan Akkawi, and Luke and Julia Barham; also survived by numerous loving nieces and nephews. The family will receive friends at Kurtz Memorial Chapel, 65 Old Frankfort Way, Frankfort, IL. 60423 on Friday, October 7, 2016 from 1:00 PM to 9:00 PM.
