Age 39, of Fort Myers FL, formerly of New Lenox, passed away on Wednesday, September 2nd, 2015 in Fort Myers, FL.

Shelly is survived by her loving family, parents, Robert & Barbara Smolka (Lepacek), children, Kaylee Anderson, Tyler Anderson, sister, Cheryl (Robert) Toler, grandfather, Raymond Lepacek, niece, Jodi Zanelli, great grandnephew, Colton Matthew Loehman and numerous aunts, uncles and cousins.

Shelly was preceded in death by her grandparents, Rosaline Lepacek (Henry) and Harry & Edwina Smolka (Maslo).

Family will receive friends at Kurtz Memorial Chapel, 102 E. Francis Road, New Lenox, IL 60451 on Thursday, September 10th, 2015 from 4:00 PM until 8:00 PM. Funeral service, Friday, September 11th, 2015 with prayers at the funeral home at 9:30 AM and then to St. Jude Catholic Church, 241 W. 2nd Avenue, New Lenox, IL 60451 for a Mass of Christian Burial at 10:00 AM.
