Age 58, of Palos Heights formerly of Frankfort and the Roseland area passed away Tuesday, August 2, 2011 at her home. She was CEO of Health PCP in Tinley Park.

Judith is survived by her loving family, children Andrew (Melissa) Haustein and Robin (Tim) Ersfeldt; grandchildren Brianne Haustein and Benjamin Ersfeldt; father Gene Bouck; siblings Jerry (Chris) Bouck, Gene (Sue) Bouck; Jane (Chad) Gilbert and Joan Young; longtime companion Joe Maschek and many nieces and nephews.

She was preceded in death by her mother Loretta Bouck (nee Bastianelli)

Family will receive friends at Kurtz Memorial Chapel 65 Old Frankfort Way Frankfort, IL 60423, Saturday, August 6, 2011 from 3 to 8 PM. Funeral service, Sunday, August 7, 2011 at 1:00 PM at All Saints Lutheran Church, 13350 S. Lagrange Ave. Orland Park, IL, 60462 Interment will be private. In lieu of flowers memorials to the Cancer Support Center 19250 Everett Ln. Mokena IL 60448 would be appreciated.
