Jason Michael Clements, 22, of Lafayette, IN, died suddenly on Sept. 14, 2018, shortly before his 23rd birthday. He is survived by his loving family: Mom: Lisa Clements (nee Reitz); Dad: Joel Clements; Birth-father: Jason Siwinski; Sister: Paige Clements; Grandparents: Charles and Donna Conway, Howard and Carole Clements, Ed and Vicki Ipema; great-grandparents: Peter and Joyce Reitz; Aunts and Uncles: his Godmother , Susan (Joseph) Lundgren, Rachael (Andrew) Maiden, Claudia Wolfe, Gary Clements, Larry (Aimie) Siwinski, Heather (Eamon) Quinn, Bob Conway, Tim (Ellen) Conway; Cousins: Joseph and Michael Lundgren, Samantha, Tamra Maiden, Cameron, Garrett Wolfe, Luke, Jacob Siwinski, Kinley, Kennedy, Emery Quinn,Kiley, Cassidy Conway, Gabriel, Jacob, Vivian Conway, Christopher (Sara), Logan, Lilly, Jeffery, (Chie), Maya, Emiko Cogswell, Elizabeth, Everett Reitz, Kayti (Chris), Kayl, Mavryk Gonzalez, Mikey, Lexi Ipema, and Friends. He was preceded in death by his grandpa, Michael Reitz. Jason was born on Sept. 27, 1995, in Palos Heights, IL and lived in the South-Suburbs of Chicago Illinois before moving when he was four years old to Lafayette, IN, where he was raised. A graduate of McCutcheon High School in Lafayette, he played trumpet in the Jazz and Symphonic bands and was a member of the Track and Field Team, specializing in the 400 meter and 4 x 4 relay. He had a passion for cars, especially Corvettes and Mustangs, he was working for Auto Express in Lafayette as a detailer. Visitation will be held on Friday, Sept. 21, 2018 from 2:00 p.m. until 9:00 p.m. and on Saturday, Sept. 22nd from 10:00 a.m. until time of Funeral Service at 12:00 p.m. at Kurtz Memorial Chapel, 65 Frankfort Way in Frankfort, IL .
